This paper describes a Systematic Human Error Reduction and Prediction Approach (SHERPA) which is intended to provide guidelines for human error reduction and quantification in a wide range of human-machine systems. The approach utilizes as its basis current cognitive models of human erformance. The first module in SHERPA perform tasks and human error analyses which identifies likely error modes, together with guidelines for the reduction of these errors by training, procedures and eqipment redesign. The second module uses the SARAH approach to quantify the probability of occurence of the errors identified earlier, and provides cost benefit analyses to assist in choosing the appropriate error reduction approaches in the third module. This implements the error reduction guidelines of the first module, and concludes with a quality assurence phase to verify their effectiveness. |
